How to Check Eligibility

We want to make it easy for you to access information you need using two convenient options to verify member eligibility:

Option 1: Call Provider Services

• Dial 800-261-3371

• Follow the prompts to check eligibility or claims status.

• To verify eligibility, you must have the members last name, first name, date of birth, and the member ID card number.

Option 2: Access the MFC-DC Provider Web Portal

• Visit: https://mfcdcprovider.healthtrioconnect.com

• Log in using your provider credentials.

• Navigate to:

  • Eligibility Search

  • Claims Status Inquiry to review claim details and processing updates.
